Select a site name by clicking on the drop-down menu under Column 2 (Block 1) and choosing one of the available options. Repeat this process until all used Training Program/Site combinations used in EXP-2 are present.
For each row, enter the number of "Principal" trainees in the profession and discipline listed.
Principal trainees are those who participated in your training program. These are the individuals reported on the Individual Characteristics (IND-GEN) form.
Do not enter counts for faculty, site staff, or other non-trainees.
In Column 5 (Block 8), enter the number of "other interprofessional" trainees who participated in team-based care alongside the Principal trainees. Other interprofessional trainees should be those who train along side the Principal trainees.
FIRST EXAMPLE:
At Site A, there were 24 preventive medicine/public health residents in your public medicine/public health residency program. These principal trainees did not train with anyone from a different training program or with any other professions. In this scenario, you would create one row for the preventive medicine/public health residents at Site A and then report the 24 preventive medicine/public health residents in column 4 and 0 in column 5.
SECOND EXAMPLE:
At Site B, 3 preventive medicine/public health residents in your preventive medicine/public health residency program received experiential training. The 3 preventive medicine/public health residents trained alongside 5 preventive medicine/public health residents and 3 internal medicine residents who were not part of your preventive medicine/public health residency program. In this example, you would need to create two rows for Site B; one row to capture the preventive medicine/public health residents and one row for the internal medicine residents.
