Setup Forms - Introduction

This page describes all setup forms. Your report will only include setup forms that apply to your program.
Setup forms must be complete before you work on other sections of the Annual Performance Report (APR). As the name implies, data and information from these forms are used to setup the rest of your report. If you fail to complete the setup forms, you may not have the data you need to complete other forms or you may not see the forms you need. Setup forms include:
- Grant Purpose Setup Form - This form captures information about the types of activities conducted by grantees of multipurpose or hybrid programs during the reporting period. The grant purpose(s) you select dictate the data entry forms you are required to complete for the reporting period.
- Training Program Setup Form - This form captures general information about the types of training programs that were supported with BHW funds during the reporting period. You must complete this setup page for each training program that was offered during the reporting period that was supported with BHW funds. This is also where you indicate if a program ended or was inactive for the reporting period.
- Faculty Development Setup Form - This form captures information about the specific types of faculty development activities conducted by grantees using BHW funds. You must select the type(s) of faculty development activities that took place during the reporting period and were supported with BHW funds to see the associated faculty development data entry forms.
Your program may use some or all of these forms.
