FIRST EXAMPLE: At Site A, there were 24 public health dentistry residents in your public health dentistry residency program. These principal trainees did not train with anyone from a different training program or with any other professions. In this scenario, you would create one row for the public health dentistry residents at Site A and then report the 24 public health dentistry residents in column 4 and 0 in column 5.

SECOND EXAMPLE: At Site B, 3 public health dentistry residents in your public health dentistry residency program received experiential training. The 3 HRSA-supported public health dentistry residents trained alongside 5 public health dentistry residents and 3 general dentistry residents that were not associated with the HRSA grant. In this example, you would need to create two rows for Site B; one row to capture the public health dentistry residents and one row for the general dentistry residents.
· In the first row, you would report the 3 public health dentistry residents associated with your public health dentistry residency program in Column 4 and the 5 public health dentistry residents not associated with your public health dentistry residency program in the same row in Column 5.
· Then, you would create a second row to report the general dentistry residents. Because none of them were part of your public health dentistry residency program You would enter 0 for column 4 and 3 in column 5.
