Item F.2: Death of Borrower: Report only the information for loans made on or after July 1 of prior year. Under the appropriate column headings, report the current number of borrowers and the amounts of principal and interest that were approved for cancellation by the institution due to death. The institution must report each write-off in the comments section of this form. The comments section should include the following information: borrowers unique ID, amount of the original loan, total amount  written off, and the reason for cancellation – uncollectable debt due to death.

Item G: Bad Debts Approved for Write-off by DHHS: Report amounts for write-off and any subsequent collections that are received and has been approved for write-off by HHS. “The institution must also report each write-off in the comments section of this form. The comments section should include the following information: borrowers unique ID, amount of the original loan, total amount written off, and the reason for cancellation – uncollectable write-off due to bad debt.
