The Office of Management and Budget (OMB) has issued a 3-year approval for the annual collection of performance measures across all grants and cooperative agreements funded through the Health Resources and Services Administration's (HRSA's) Bureau of Health Workforce (BHW) (OMB # 0906-0086; Expiration Date: 03/31/2027). Your Annual Performance Report (APR) is updated with all approved forms and columns which you can see in subsequent sections of this manual. Throughout the manual, there are several icons that identify tips and other important information that will assist you in completing each required form accurately (see below).

Marks a warning statement. Please read information in bold carefully in order to complete each subform accurately.

Marks a tip or important note for completing a specific column or form in the Annual Performance Report (APR).

 Marks the end of a form and provides instructions for initiating required validations checks (in other words, checking the form for errors).

Due to the nature of annual reporting, the Annual Performance Report (APR) prepopulates specific columns within certain forms with data from the previous reporting period. In addition, a "View Prior Period Data" link is included at the top of each form to provide you with easy access to data submitted by your organization in prior reporting periods.
