Structured Development Activity (FD 1a FD 1b) – Please enter each course that you, the Project Director, have taken as a series of curriculum-based training activities for your career development, that lead towards earning a degree, a certificate, or/and a fellowship, for example an MPH degree, a doctorate degree.  Structured faculty development programs can be degree bearing or certificate bearing and are not necessarily offered at the grantee institution or organization. 

Faculty Development Activity (FD 2a FD 2b)– Please enter each course that you, the Project Director, have taken as a stand-alone single training activity or event or workshop you took for the purpose of additional training for your career development.  Faculty development activities may include conferences or grand rounds and are not necessarily offered at the grantee institution or organization.

Faculty Instruction (FD 4a FD 4b)– Please enter each course, workshop, seminar, grand rounds or other training activity that you developed and taught to faculty/health care providers.  Please include courses that other faculty taught to faculty and/or other health care providers, as a result of the grant.  Please do not repeat courses entered onto Forms FD-4a  onto Forms CDE-1 .
