Select Type of Training Program Offered: Select the type of training program supported through the grant during the current reporting period by clicking on the drop-down menu and choosing one of the following options: - Degree/Diploma/Certificate Academic Training Program (Degree/Diploma)
- Fellowship program
- Internship Program
- Practicum/Field Placement program
 Note Professional Track Grantees- Select "Fellowship," "Internship Program," or "Practicum/Field Placement Program". Definitions are as follows: - Fellowship—a postdoctoral training component that provides advanced clinical or research training to a graduate from a health professions program in a specialized area. Participants have earned a terminal degree such as a Ph.D. or M.D. prior to entrance. Fellowships are usually accredited by a recognized accreditation agency.
- Internship—a type of training activity that may be predoctoral or postdoctoral and may be a component of a degree-bearing program. For health professions programs, internships provide significant clinical training in a health care setting. For doctoral programs in clinical, counseling and school psychology, internships are an accredited 12-month supervised experience in a health care setting.
- Practica/Field Placement - a type of experiential training activity supervised by a faculty member as part of an accredited degree, certificate, or organized educational program. Experiential training activities provide direct work experience.

Select Delivery Mode Used to Offer Program: Select the primary mode used to deliver your certificate program during the current reporting period by clicking on the drop-down menu under and choosing one of the options from the list below. - Click on the "Add Record" button to save your entry.
- Repeat this process to capture the degree programs of all students who received a BHW-funded financial award during current reporting period.
- Campus-based program
- Distance learning program
- Hybrid program
Warning The primary focus option "Student- Paraprofessional- Other" should only be selected if none of the provided options appropriately describes your paraprofessional certificate program. Please contact your government project officer to confirm that this is the appropriate option to select. If you select "Other," leave a comment in your report describing what the primary focus should be and let your project officer know the option is not available. |

Select Training Activity Status in the Current Reporting Period: Select the status of each internship, practicum/field placement, fellowship, or degree/diploma/certificate program at the end of the current reporting period i.e., June 30, 2026) by choosing one of the options below: Note Active programs- A training program that was offered during the current annual reporting period. If you are reporting any activity for a program, please select active.
Note Inactive Programs- A training program that was NOT offered during the current annual reporting period. Selecting ‘Inactive’ indicates that the training program is completed, you are no longer administering it, and you have no active INDGEN records or faculty development programs. You will not report on any aspect of an inactive program, and all records associated with the program (i.e., EXP records and CDE records) will be made inactive. To Complete the Form: Click on "Save and Validate" on the bottom right corner of your screen. If no errors are found, the system will automatically route you to the next required subform. |
