The SOHWP forms captures additional information about the types of activities conducted by the grantees during the current reporting period.

• SOHWP-A form captures information about new dental facilities in underserved areas established by the grantee
• SOHWP-B form captures information about existing dental facilities expanded by the grantee in underserved areas
• SOHWP-C form captures information about teledentistry education training offered by the grantee.
• SOHWP-D form captures information about the types of community-based preventive services provided by the grantee.
• SOHWP-E form captures information about programs for children that encourage future careers in oral health and science professions provided by the grantee.
• SOHWP-F form captures information about hiring and retention of state-level oral health-related staff.
• SOHWP-G form captures information about other oral-health related activities provided by the grantee.
