Per the NOFO, please note that the NRSA program does NOT support faculty development activities. Therefore, these forms should be completed with information related to FELLOW development programs, activities and instruction.

Select Profession of Faculty/Fellow Trained: Select the individual’s profession by clicking on the drop-down menu next to "Select Profession of Faculty Trained" and selecting one of the available options.
Select Discipline/Specialty of Faculty/Fellow Trained: Select the individual’s discipline/specialty by clicking on the drop-down menu next to "Select Discipline/Specialty of Faculty Trained" and choosing all that apply from the available options. Please note that you must select disciplines/specialties that are associated with the profession you selected above. The disciplines/specialties are organized by profession in the drop-down; please scroll to find the correct option.
Activity Name: To add information about the profession(s) and discipline(s) of faculty/fellows who participated in each activity during the annual reporting period, select an activity by clicking on the drop-down menu next to "Activity Name" and choosing
one of the available options.

- Dentistry
- Medicine
- Nursing
- Other

- Physician Assistant
- Public Health
Note
Information regarding the names of faculty development activities will prepopulate with the information that was entered and saved in the FD-2a subform.
Example:
If you are reporting on both Pediatricians and Dermatologists, you would select "Medicine" for the Individual's Profession and then select both "Pediatrics" and "Dermatology" from the Individual's Discipline/Specialty drop-down list.
Example: If you are reporting on an individual who is a Plastic Surgeon, the Individual’s Profession would be “Medicine” and the Individual’s Discipline/Specialty would be “Plastic Surgery”.
Example: If you are reporting on an individual who is studying to be an Occupational Therapist, the Individual’s Profession would be “Allied Health – Student” and the Individual’s Discipline/Specialty would be “Occupational Therapy”.
Example: If you are reporting on an individual who is a Clinical Social Worker, the Individual’s Profession would be “Behavioral Health” and the Individual’s Discipline/Specialty would be “Clinical Social Work”.
