FD - Introduction

Purpose of FD forms:

• The FD-1 subforms collect information about the characteristics and the trainees of structured faculty/fellow development programs. 
• The FD-2 subforms collect information about the characteristics and the trainees of faculty/fellow development activities. 
• The FD-4 subforms collect information about the characteristics of faculty/fellow-instructed courses.

Per the NOFO, please note that the NRSA program does NOT support faculty development activities. Therefore, these forms should be completed with information related to FELLOW development programs, activities and instruction.

The FD-1a and FD-1b subforms will only appear if "Structured Faculty/Fellow Development Training Program" was selected in the Faculty/Fellow Development Setup Form.

The FD-2a and FD-2b subforms will only appear if "Faculty/Fellow Development Training Activity" was selected in the Faculty/Fellow Development Setup Form.
