Setup Forms - Introduction
Setup forms must be complete before you work on other sections of the Annual Performance Report (APR). As the name implies, data and information from these forms are used to setup the rest of your report. If you fail to complete the setup forms, you may not have the data you need to complete other forms or you may not see the forms you need. Your setup forms include:
- Training Program Setup Form - This form captures general information about the types of training programs that were supported with BHW funds during the reporting period. You must complete this setup page for each training program that was offered during the reporting period that was supported with BHW funds. This is also where you indicate if a program ended or was inactive for the reporting period.
- Faculty Development Setup Form - This form captures information about the specific types of faculty development activities conducted by grantees using BHW funds. You must select the type(s) of faculty development activities that took place during the reporting period and were supported with BHW funds to see the associated faculty development data entry forms.

Please be sure to update the setup forms section of your report every year before proceeding to other forms.
