PC Subforms - Introduction

Warning

Your program manual shows all possible PC forms for the grant program. However, PC forms show up on your APR based on the types of training programs you enter under Training Program Setup. If you have a training program to report and do not see the associated PC form, please return to the Training Program Setup form and add it. If it is already there, make sure it has a "training activity status" of active, ongoing, or complete.

If there are no questions to answer on your PC form (in other words there are no editable fields), please click save and validate at the bottom of your screen to move on to the next subform.

The Program Characteristics (PC) subforms are designed to collect additional information about training programs that were offered during the performance period. Each of the PC subforms corresponds to a different type of training program. You may have one or more of the following:

• PC-1 - Degree/Diploma/Certificate Programs
• PC-2 - Unstructured Training Programs (non-degree)
• PC-3 - Structured Training Programs (non-degree)
• PC-4 - Internship Programs
• PC-5 - One Year Retraining Programs
• PC-6 - Fellowship Programs
• PC-7 - Practica and Field Placement Programs
• PC-8 - Residency Programs
• PC-9 - Position Descriptions for Residency and Fellowship Programs
• PC-10 - Major Participating Sites/Rotation Sites

If the form requests counts, include data about all program participants, not just participants supported or trained by the Bureau of Health Workforce (BHW) grant program. In other words, the total count for the training program should include those who received BHW financial support or training as well as those who did not - for example, all students enrolled in the degree program or all residents in the residency program (see detailed examples below). 

Please complete the required subforms for each program that was entered in the Training Program Setup form. If you have any questions about how to complete this form, please refer to the instruction manual and/or contact your Government Project Officer.

Note

The PC forms pre-populate with the information you entered about the training programs under Training Program Setup. The only training programs that will not show up are ones marked as "inactive" on the Training Program Setup form.

Note

 If your PC form requires counts consider the following examples:

• You have a training program with 10 students, and 8 received financial support or training through your BHW grant program, but the other 2 did not receive any BHW financial support or training. In this case, you would report all 10 students on your PC form, but you would only report on the 8 BHW-supported or trained students for the remainder of the APR.

• You have a training program with 10 students, and 8 received financial support through a BHW grant program. The other 2 did not receive financial support, but they did participate in training that was funded in whole or in part by the BHW grant program. In this case, you would report all 10 students on your PC form and throughout your APR.

Note

 If you wish to view data that were submitted in the prior reporting period, click on the ‘View Prior Period Data’ link and a read-only version of your most recent prior performance report will pop-up in a new screen. Also, records about ongoing training programs or activities from the prior reporting period will auto-populate into this form and are identified as ‘Prior Records’ under the column labeled "Record Status".
