The FD-1a and FD-1b subforms will only appear if "Structured Faculty Development Training Program" was selected in the Faculty Development Setup form.
Multiple steps are required to complete this portion of the subform. Please read instructions carefully.
Enter the name of each new structured faculty development program coordinated and/or supported through the grant during the current reporting period. Next, select "Add Record." Repeat this process as necessary to enter each new structured faculty development program that was coordinated and/or supported through the grant during the current reporting period.
Select "Ongoing" if the training program did not conclude by the end of the current reporting period.
Select "Complete" if the training program concluded at some point during the current reporting period.
Select "N/A" if the program culminates in a degree that is not in dentistry, education, or public health.
For Column 5 (Block 3): It should be hours per offering in the reporting period. For example, it the course is offered once per year (3 hours per week for 15 weeks), then the entry should be 45.
Select whether any faculty who participated in a structured faculty development program and received any type of BHW-funded financial award during the current reporting period by clicking on the drop-down menu in Column 11 (Block 7) and choosing from the following options:
