Select Profession of Faculty Trained: Select the individual’s profession by clicking on the drop-down menu next to "Select Profession of Faculty Trained" and selecting one of the available options.
Select Discipline/Specialty of Faculty Trained: Select the individual’s discipline/specialty by clicking on the drop-down menu next to "Select Discipline/Specialty of Faculty Trained" and choosing all that apply from the available options. Please note that you must select disciplines/specialties that are associated with the profession you selected above. The disciplines/specialties are organized by profession in the drop-down; please scroll to find the correct option.
Information regarding the names of faculty development activities will prepopulate with the information that was entered and saved in the FD-2a subform.
Only faculty should be reported on this form. Please report students on CDE forms and practicing professionals/practicing paraprofessionals on the CE forms (if applicable).
If you are reporting on both Pediatricians and Dermatologists, you would select "Medicine" for the Individual's Profession and then select both "Pediatrics" and "Dermatology" from the Individual's Discipline/Specialty drop-down list.
Enter the number of faculty in each profession/discipline who participated in the faculty development activity during the current reporting period in the textbox in Column 3 (Block 12). Repeat this step as many times as necessary to capture the total number of faculty by profession/discipline who participated in each faculty development activity during the current reporting period.
