Multiple steps are required to complete this portion of the subform. Please read instructions carefully.
THCPD programs that have residents/fellows in INDGEN must report them in EXP-2. THCPD programs that have not yet started training residents/fellows can leave this form blank.
Select a training program by clicking on the drop-down menu in Column 1 and choosing one of the training program options.
Pair the selected training program with a training site by clicking on the drop-down menu in Column 2 and choosing a site name. The options available will be sites that were marked as "Used" in the current reporting period on EXP-1.
In Column 5 (Block 8), enter the number of "other interprofessional" trainees who participated in team-based care alongside the Principal trainees. Other interprofessional trainees should be those who train along side the Principal trainees.
FIRST EXAMPLE: At Site A, there were 24 family medicine residents in your family medicine residency program. These principal trainees did not train with anyone from a different training program or with any other professions. In this scenario, you would create one row for the family medicine residents at Site A and then report the 24 family medicine residents in column 4 and 0 in column 5.

SECOND EXAMPLE: At Site B, 3 family medicine residents in your family medicine residency program received experiential training. The 3 family medicine residents trained alongside 5 family medicine residents and 3 internal medicine residents who were not part of your family medicine residency program. In this example, you would need to create two rows for Site B; one row to capture the family medicine residents and one row for the internal medicine residents.
· In the first row, you would report the 3 family medicine residents from your family medicine residency program in Column 4 and the 5 family medicine residents who were not part of your family medicine residency program in the same row in Column 5.
· Then, you would create a second row to report the internal medicine residents. Because none of them were part of your family medicine residency program, you would enter 0 for column 4 and 3 in column 5.
