EXP - Introduction

1. Purpose: The EXP forms are used to capture experiential or clinical training characteristics, including training sites and the number and profession/discipline of trainees.

• The EXP-1 Setup form captures information about the sites used by grantees to provide trainees with in-person clinical or experiential training
• The EXP-2 form collects information about the profession and discipline/specialty of individuals trained at each site that was entered in the EXP-1 Setup form along with interprofessional trainees.

2. Order of Forms:

• EXP forms MUST be completed in order, otherwise drop-down menus will not prepopulate correctly.
• You must complete and ‘Save and Validate’ EXP-1 first before proceeding to EXP-2.

3. Pre-population of Prior Records (training sites) reported previously:

• The  system will prepopulate the site names and some of the site details for every site you reported last year on the EXP-1 form.
• In Column 2, 'Select Whether the Site was used in the Current Reporting Period', indicate whether each previously-reported site was used during the current annual reporting period.
  • If 'Yes', the system will include the site on the EXP-2 form and you will be prompted to enter the number trained at the site by profession and discipline/specialty.
    If 'No', the system will not include the site on the EXP-2 form.

Warning

Complete the EXP-1 and EXP-2 forms only for sites used to train individuals who appear on the INDGEN form. 

Warning

Only include clinical training sites; not virtual sites or simulation-based training sites. Virtual or simulation-based training should be reported on the CDE forms.

Note
 New sites should only be entered if the site was used during the current reporting period for in-person experiential training.

EXP - Standardizing Addresses for Active Sites

You must first complete address standardization before working on EXP-2.

Note

You should accept the standardized address unless it is incorrect, in which case you can select the option to keep the address as you entered it. Please first confirm the address you entered is indeed correct and the one where the site receives mail before choosing this option.

If your screen shows up blank with no records, then all addresses are standardized. You can then click save and continue to move on to EXP-2. 

EXP - FAQs

Question 1: For the EXP-1 form, should we list all sites that we have entered into our QPUs or, only sites grantees use to provide trainees with in-person clinical or experiential training?

Answer: For the EXP-1 form, grantees should only report sites used to provide in-person training to students, trainees, or residents/fellows supported by the grant during the current reporting period.

Question 2: Are the EXP-1 and EXP-2 form only for trainees (students/residents/fellows) within a training program, or should we also list all clinical training sites where we have trained working professionals who are only listed on the continuing education forms?

Answer: Please include training sites that provide in-person experiential training to trainees who are on the IND-GEN form.

Question 3: What if the students train in three to four unique entities to complete their Longitudinal Clinical Rotation?

Answer: Please include all training sites that provide in-person, experiential training to trainees who are on the IND-GEN form on the EXP-1 form. If the trainee participated in multiple rotations during the performance period, please report all training and contact hours the trainee received into the trainee’s row in Column 27d, 27e, 28, 29, 31, 32, 33, and 34 on the IND-GEN form.

NOTE: Students should not train at three to four unique entities for their longitudinal clinical rotations.  For HRSA-24-018 a longitudinal clinical rotation means a continuous clinical immersion in TTOUR primary care sites/delivery systems for a duration of one eight-week or two four-week periods within an academic year. The rotations must be at least twenty hours per week per trainee.
