Select Profession of Individuals Trained: Select the individual’s profession by clicking on the drop-down menu next to "Select Profession of Individuals Trained" and selecting one of the available options. Select Discipline/Specialty of Individuals Trained: Select the individual’s discipline/specialty by clicking on the drop-down menu next to "Select Discipline/Specialty of Individuals Trained" and choosing all that apply from the available options. Please note that you must select disciplines/specialties that are associated with the profession you selected above. The disciplines/specialties are organized by profession in the drop-down; please scroll to find the correct option. Example: If you are reporting on both Pediatricians and Dermatologists, you would select "Medicine" for the Individual's Profession and then select both "Pediatrics" and "Dermatology" from the Individual's Discipline/Specialty drop-down list.
Example: If you are reporting on an individual who is a Plastic Surgeon, the Individual’s Profession would be “Medicine” and the Individual’s Discipline/Specialty would be “Plastic Surgery”.
Example: If you are reporting on an individual who is studying to be an Occupational Therapist, the Individual’s Profession would be “Allied Health – Student” and the Individual’s Discipline/Specialty would be “Occupational Therapy”. Example: If you are reporting on an individual who is a Clinical Social Worker, the Individual’s Profession would be “Behavioral Health” and the Individual’s Discipline/Specialty would be “Clinical Social Work”. |

Enter # Trained in this Profession and Discipline: For each course, enter the number of participants from the professions/disciplines you have indicated under Column 3 (Block 7). Repeat this process to capture the profession and discipline of all individuals trained in each course or training activity during the current reporting period. Once you have completed Columns 1 through 3 for all rows, click ‘Save and Validate’ to automatically populate Columns 4 through 9 with responses from CDE-1. |
