Enter Total # Enrolled (whether funded by BHW or not): Total: For Column 4 (Block 3), enter the total number of individuals in the profession and discipline who participated in a fellowship during the current reporting period. Count all individuals who participated in a fellowship—regardless of whether they received a BHW-funded financial award or not (this is a total enrollment count of fellows in the fellowship program except those who permanently left the fellowship program).Enter Total # Enrolled (whether funded by BHW or not): URM: For Column 5 (Block 3b), enter the number of individuals who participated in a fellowship during the current reporting period who were not underrepresented minorities.Enter Total # Enrolled (whether funded by BHW or not): Disadvantaged Background and not URM: For Column 6 (Block 3b), enter the number of individuals from disadvantaged backgrounds who participated in a fellowship during the current reporting period who were underrepresented minorities.

Note

For the total enrollee count (Column 4), DO include students who went on to complete the fellowship program in the current reporting period. Do NOT include students who discontinued prior to program completion (i.e., dropped out). Drop-out counts will be captured separately in Block 9.

Note

Columns 5 and 6 are subsets of Column 4.

Note

You must report every individual that received direct financial support (e.g., stipends) from the HRSA-funded grant, and all non-HRSA funded trainees enrolled in the Addiction Medicine Fellowship program. The total trainees reported on the PC-6 form may match the total trainees reported on the INDGEN form.

Example: The School of Medicine had a total of 12 addiction medicine fellows in the fellowship program during the current reporting period. The school used BHW funds to provide stipends to 5 out of the 12 fellows. During this period, 1 fellow left the program before completing all fellowship requirements.
In Column 4 (Block 3) of this form, the reporting official at the School of Medicine would enter 11

Example:

Example: The School of Medicine had a total of 11 addiction medicine fellows maintain enrollment in the fellowship program during the annual reporting period. Among the 11 fellows who were enrolled in the program, a total of 3 fellows completed all fellowship requirements during the reporting period.
In Column 7 (Block 8), the reporting official at the School of Medicine would enter 3.

Note
Column 10 is a subset of Column 9. The total entered in Column 9 is exclusive of the total number of enrollees previously entered.

Example: The School of Medicine had a total of 12 addiction medicine fellows in the fellowship program during the current reporting period. The school used BHW funds to provide stipends to 5 out of the 12 fellows. During this period, 1 fellow left the program before completing all fellowship requirements; none who left were underrepresented minorities.
In Block 9 of this form, the reporting official at the School of Medicine would enter 1.
In Block 9a of this form, the reporting official at the School of Medicine would enter 0.
