This instruction manual has been carefully designed to assist your organization in completing the required Annual Performance Report (APR). Please read through this manual carefully, as it contains examples and a series of step-by-step instructions that will aid you in completing all required forms. A few things to note:






1. All HRSA awardees are required to collect and report performance data so HRSA can meet its obligations under: The Government Performance and Results Modernization Act of 2010, and the Foundations for Evidence-Based Policymaking Act of 2018. This data collection is statutorily required and approved by the Office of Management and Budget.

 

2. All required performance measures are linked to the following legislative purpose(s) of the MATCARE grant program:



The purpose of Maternity Care Nursing Workforce Expansion (MatCare) Program is to grow and diversify the maternal and perinatal health nursing workforce through support for education and training. The program seeks to increase and diversify the number of Certified Nurse Midwives (CNMs), and prepare them to serve in rural, urban, and tribal underserved communities nationwide with a focus on addressing and reducing maternal mortality and morbidity. The program will fund accredited nurse midwifery programs to award scholarships to students in the nurse midwifery program to cover tuition costs. In addition, the program will also support costs for each project to administer the training.



3.  Data submitted by grantees must cover all activities that took place between July 01, 2025 - June 30, 2026. This is true for all grant programs except the National Health Service Corps - State Loan Repayment Program (NHSC-SLRP) which must report data covering all activities that took place between September 01, 2025 - August 31, 2026

 


4.  Each year, the APR is due no later than July 31, 2026. Failure to submit an Annual Performance Report (APR) by this date may place your grant in a noncompliant status. If you are reporting for National Health Service Corps - State Loan Repayment Program (NHSC-SLRP), the Annual Performance Report (APR) is due no later than September 21, 2026




5.  Officials at the Health Resources and Services Administration (HRSA) will review and approve all Annual Performance Reports (APRs) submitted by grantees. In the case that revisions are needed, you will be granted the ability to re-enter your Annual Performance Report (APR), make corrections, and submit a revised Annual Performance Report (APR). All revisions must be resubmitted within five (5) business days of the initial request. Failure to resubmit a revised Annual Performance Report (APR) within five (5) business days may place your grant in a noncompliant status.
