· Community Prevention Track: o Fellowship: For programs training residents/physicians in the specialty of public health and general preventive medicine, please report them as “Fellows” in a Medicine – Preventive Medicine Fellowship Program. o Residency: For programs training resident physicians in a combined internal medicine-preventive medicine or family medicine-preventive medicine residency program, please report them as “Residents” in either a Medicine – Preventive Medicine/Internal Medicine or Medicine – Preventive Medicine/Family Medicine Residency Program. · Obstetrics Track: o Fellowship: If you are training physicians that have completed a residency and are board-eligible or board-certified in family medicine and are training in the second specialty of obstetrics, please report them as “Fellows” in a Medicine – Obstetrics and Gynecology Fellowship Program. o Residency: If you are training resident physicians in a family medicine residency program receiving enhanced obstetrical training provided by a co-located obstetrics-gynecology residency program, please report them as “Residents” in a Medicine – Family Medicine Residency Program. Select Type of Training Program Offered: The Training Program Setup form will configure all subforms specific to the residency or fellowship program. You will need to add a record for each residency and/or fellowship program that is supported with grant funds. To begin completing the setup for new records, select the type(s) of training program(s) supported with grant funds during the current reporting period by clicking on the drop-down menu next to “Select Type of Training Program Offered” and choosing one of the following options.- Fellowship program

To complete the Training Program Setup form, review the Saved Records Table to ensure that all training programs or training activities supported with grant funds during the current reporting period were captured accurately. For new records, please review the information contained in the table for accuracy and if a record has to be deleted, simply click on the "Delete" link under the Option(s) column. Select Training Activity Status in the Current Reporting Period: Select the Training Activity Status of all reported training programs. If you are reporting on a program, please choose ‘Active.’ To Complete the Form: Click on "Save and Validate" on the bottom right corner of your screen. If no errors are found, the BPMH system will automatically route you to the next required subform. |
