FD-2b - Adding Profession and Discipline/Specialty for Activities

Select Profession of Faculty Trained: Select the individual’s profession by clicking on the drop-down menu next to "Select Profession of Faculty Trained" and selecting one of the available options.

Select Discipline/Specialty of Faculty Trained: Select the individual’s discipline/specialty by clicking on the drop-down menu next to "Select Discipline/Specialty of Faculty Trained" and choosing all that apply from the available options. Please note that you must select disciplines/specialties that are associated with the profession you selected above. The disciplines/specialties are organized by profession in the drop-down; please scroll to find the correct option.

Note

Information regarding the names of faculty development activities will prepopulate with the information that was entered and saved in the FD-2a subform.

Note

 Only faculty should be reported on this form. Please report students on CDE forms and practicing professionals/practicing paraprofessionals on the CE forms (if applicable).

FD-2b - Entering # Trained in the Profession and Discipline

To Complete the Form: Click on the “Save and Validate” button located on the bottom right corner of your screen. If no errors are found, click on the "Submit" button in the left hand side of the EHB to begin the submission process of your PRGCA.
You have not submitted your PRGCA until you receive a message indicating that your report has been successfully submitted. Please follow instructions on the next few pages. You must validate your report, and finally the report must be certified by the submitting official at your institution before it is submitted to HRSA. Please ensure that this process is completed in a timely manner.
