To configure the system, complete the Grant Purpose Setup form by selecting the specific type(s) of activities that were supported with grant funds during the reporting period (July 01, 2025 - June 30, 2026). Some options in the Grant Purpose form will be automatically selected based on information provided in a previous reporting period.
AHEC-1: Health careers recruitment of underrepresented minority populations or individuals from disadvantaged or rural backgrounds: For this grant purpose, please complete the following forms:

- LR-1a
AHEC-2: Community-based training and education with emphasis on primary care: For Community-based Education and Training (CBET) placed by the Program Office or Centers, this is considered a field placement/practicum for the APR. Please complete the following forms:

For CBET developed and administered by partners, please complete the following forms**:

AHEC-4: Public health careers exposure to youth: For this grant purpose, which includes youth pipeline/pathways activities such as those related to developing high school students (grades 9-12) please complete the following forms:

Unselecting a grant purpose will cause related forms and data to be deleted. If you need to unselect a grant purpose, contact your project officer first to ensure you do not unnecessarily lose any data.
To Complete the Form: Click on "Save and Validate" on the bottom right corner of your screen. If no errors are found, the BPMH system will automatically route you to the next required subform.
